How to Enroll

Documents needed for registration:


​Parent or guardian ID
- Please present to the main office, a copy will be placed in your child's enrollment file. 

Student birth certificate - Bring an original copy, we will keep a photocopy in your child's file and return the original.  A state issued passport is also acceptable if the original birth certificate is not available. 

​Proof of address - Even if you are being accepted outside our attendance area, we still need a current proof of residency.  Acceptable forms include a utility bill, lease agreement, or current voter registration card. 

​Proof of income (Preschool only) - This is required for income verification for preschool registration.  Acceptable documents include a paystub, 1040 or W-2, SSI letter, Unemployment letter, pension stub, or TANF documents.

Minimum Health Requirements


The health of our entire community is a priority.  Please make sure that your child is up to date on all vaccinations and has a current physical on record.  An updated physical is required for grades Preschool, Kindergarten, and 6th grade.  Any student entering CPS for the first time must also have a current physical and vaccine record.  

Proof of Dental exams are required for Kindergarten, 2nd grade, and 6th grade. 

Proof of Vision exams are required for Kindergarten and any student new to CPS.
